For Author Use Only:
At The Bottom of Your Document:
1. Put the the word "copyright" on your material.
2. Put the year you created the material.
3. After the year, put your full name.
Sample: Copyright 2012 Your Full Name
4. Declare that you are the author by writing the following:
"I, "Your Full Name" Declare Under Penalty of Perjury that
I am The Author of the Attached Material."
5. Sign the material using your signature.
6. Save your material.
Then, attach your material and e mail your material to:

It is your responsibility to maintain a copy of that transmission for your records of copyright.
It is your responsibility to correctly attach the copyright information.
It is your responsibility to be the true author of the material sent.
